Diablo 1 Walkthrough -> Black Mushroom

Once you reach level 9, already in the caves, you can find this delightful quest. The tricky part is not missing its components. Black mushrooms grow on the ground in the caves, but you won't be able to collect them until you find the Mushroom Tome and bring it to Adria the Witch.
It's convenient if you found the tome first and then the mushroom itself. Either way, Adria will tell you that she's experimenting with potions, and the book you brought her contains many wonderful recipes. For one of them, she'll need the aforementioned black mushroom.
Having found this mushroom on level 9, take it to the witch. She'll tell you she's made a wonderful potion and ask you to bring her one more component: a healer's elixir. After talking with Pepin, you'll learn he needs a Demon Brain for this elixir. Descend into the caves, kill any demon there, and they'll drop a brain, which is so essential for the healer.
Returning to town, take this elixir component to Pepin, and he'll give you a nice bottle. However, trying to return it to Adria won't be successful: she'll only irritably tell you she doesn't need it and that you can keep it.
Reward: Prismatic Elixir, which increases all hero parameters by +3.
